Closer monitoring in hypertrophic cardiomyopathy

Achieving 80% patient follow-up coverage to detect clinical deterioration between cardiomyopathy consultations

Closer monitoring in hypertrophic cardiomyopathyCloser monitoring in hypertrophic cardiomyopathy

Summary

The Family Cardiomyopathies Unit at Hospital Clínico Universitario Virgen de la Arrixaca is a reference unit caring for a large population of patients with hypertrophic cardiomyopathy. Problems don't happen on the day of the visit but between visits; symptoms are under-reported, because patients don't always recognise how serious they are, and the care burden prevents the team from seeing patients as often as needed. LOLA is integrated into the usual care pathway. It asks structured questions with a scoring system and alert levels — for example, worsening or syncope triggers a severe alert. The protocol was cocreated with Dr. Gimeno and his team, who defined what to monitor and what generates an alert.

Impact

~80%
Coverage: 8 in 10 patients completed their phone follow-up
The dynamics of consultations have changed. Patients followed up with Lola arrive better prepared, have a clearer understanding of their symptoms, and communicate their progress more accurately. This allows us to prioritise follow-up and adjust the frequency of visits according to each patient’s clinical situation.
